Dosing
- Usual ceftriaxone dose: 1g -2g IV q24h; with higher dose used for bacteremia and obesity (https://www.customid.org/policy/antimicrobial-dosing-adult-obese-patients)
- Endocarditis (treatment), osteomyelitis, bloodstream infection, serious intra-abdominal infection, other serious infection involving abscesses or indwelling hardware: 2g IV q24h
- Endocarditis (synergy with ampicillin for Enterococcus endocarditis): 2g IV q12h
- Meningitis: 2g IV q12h
- SBP: 2g IV q24h
- Uncomplicated gonorrhea: 500mg IM or IV x 1 (per MMWR Sexually Transmitted Diseases Treatment Guidelines, December 18, 2020)

Orders for ceftriaxone doses other than 1g IV q24h for CAP or UTI will be switched to 1g q24h per P&T approved protocol.

Fixed (ie non weight-based) doses in adults are historically based on a 70 kg patient. Specific disease states or individual patients may warrant dosages that differ from the above recommendations. Since product-specific criteria for dose adjustment based on creatinine clearance exist, consult product information regarding specific recommendations for dosage adjustment based on estimated creatinine clearance.
